Observation

1) One individual care plan (health plan) for a food allergy is signed by the parent, but does not include written instructions from the child's health care provider as required. The plan is not clear regarding when to administer emergency medication. 4) The food allergy individual care plan does not have documentation of having been reviewed by all staff working at this site who are responsible for serving food to children.
