0145(1): Licensor observed hazards present and accessible to children in the licensed outdoor space including a bucket and mop, a broken laundry basket with sharp edges, a broken plastic rocking horse/riding toy with sharp edges, and a long cord attached to a camera mounted on the building above the back door accessible to children and long enough to form a loop around a child's neck. 0145(11): Licensor observed the gate in the licensed outdoor play area was not self latching. 0145(8): Licensor observed a wooden slat on the gate was deteriorating and beginning to rot away. Licensor observed a gap between the bottom of the fence and the ground to be 4.5". 0145(9): Licensor observed a gap between the house and the gate to be 4.5".
