Observation

Licensor observed diaper changing area to be located in the hallway and not within arm's reach of bathroom sink. Diaper changing area was more than 5ft. from sink. Licensor did not observe a diaper disposal that is hands-free and covered with a lid anywhere near the diaper changing area.
