Observation

Medication for a child's 3 enhalers and 2 epi pens, had no written and signed consent for use from the parent. Only one medication (non expired enhaler) had the presription medication lable with it. All medications onsite had no medication authorization form that would include possible side effects of the medication. The only epi pens (2) and 2 of 3 enhalers onsite did not have a medication label with the child first and last name, each epi pens had different dates and both were expired as were , the epi pens did not have the prescbing health professional name or contact information, no dossage. The Lead Teacher reported that a student has her own enhaler in her backpack inwhich she uses the enhaler when needed but the staff have no written system or log when the medicaiton was given. No permission/consernt form observed from the parent or gaurdian. None of the medications had any logs present.
