An Asthma inhaler has no prescription box with no way to know who the medication is prescribed for. No First and last name of the child or the day the prescription was filled could be verified. No name of who prescribed the medication also missing. Medication was observed accessible to children in backpacks sitting on a table. A controlled substance medication for a child was observed in an unlocked container and accessible to children The provider corrected the violation during the onsite visit by puttings both medications in a locked cabinet.
