Observation

Asthma medication was observed without the prescription box. Inhaler was present with no indication of who it was prescribe for, the date it was prescribed, the name of the prescribing health professional, dosage amount and length of time to give the medication. No instructions for storage was observed. Permission forms have just expired for an asthma medication. Need to be be updated annually with the parent or guardian. A locked box with a controlled substance, ADHD medication, was observed in the classroom sitting on a counter top accessible to children. Provider corrected onsite.
