Observation

Licensor observed two cables in licensed playroom near refrigerator to be connected to a low outlet. Both cables were observed loose and large enough to create a loop around a child's neck. Licensor observed a roll of plastic trash bags inside of the bathroom cabinet under sink space. Cabinet did not have a child lock and bags were large enough to fit over a child's head. Licensor observed outdoor wooden train to have nails sticking out and accessible to children. Licensor observed outdoor wooden train to have broken wood on various side including inside, and on wheels. Sharp edges were obsevred and a hole large enough to fit a child's foot. Licensor observed water temperature in bathroom inside of sleeping room to be at 122 degrees Fahrenheit.
