0165(2)(c) There was some phone cords accessible in the kitchen during the monitor visit. 0165(2)(d) There was a plastic Ziploc bags with beads on top of the cubbies in the living room. The provider removed Ziplock bag with beads after licensor gave TA. There were some plastic bags accessible in the kitchen on the table during the morning visit.
