During onsite inspection, Licensor observed a play pen placed in front of a window in the licensed bedroom area (2)(g) During onsite inspection, Licensor observed three rusty nails and eight rusty screws in a wall in the side of a building structure accessible in the outdoor area (3)(a) During onsite inspection, Licensor observed a glass light bulb in use in the bathroom not meeting the shatter resistance requirement. (4)(c)(i) During onsite inspection, Licensor observed two outlets at the left and right of the kitchen sink, not meeting the GFCI outlet type. (5)(c)
