0165(2)(c) cords from canopy(tent) formed loops and were accessible to children on the playground. 0165(2)(d) plastic bags were accessible to children in the hallway bathroom 0165(4)(c) Observed rooms #1 and #2 were too dark during nap time to ensure adequate supervision 0165(4)(d) Observed noise level too high and some children appeared to be overwhelmed because 8-10 children and 2 teachers were present together in the hallway bathroom which is a confined space.
