Provider and assistants did not have diploma registered in MERIT. An assistant had not completed food handler permit, Child Care Basics, and health and safety trainings within 3 months of hire. Provider completed 20 hour initial training and did not complete health and safety trainings in emergency preparedness and mandated reporting.
