Observation

0165(2)(c) in the hallway by the entrance on the left side, an electrical cord of the radio was accessible during childcare hours, corrected on-site 7/17/2025. 0165(3)(g) in the hallway passing the school-age, a tamper resistant outlet has a gap of 1/2 inche on the right side of the plate where liquid can go inside the wall; in the bathroom, the door trim is not secure to the floor, in the large room, going to the outdoor; and on the double door, the trim has a screw that is sticking out; these three items were accessible to childcare children.
