(2)(m) - The liability statement was not posted; instead, a blank form without any indication of whether the provider carried insurance was displayed. During the visit the provider stated that she is providing liability insurence for children. (2)(d) - A diaper-changing poster was displayed near the kitchen sink instead of the required hand-washing poster in the kitchen. Handwashing posters in the bathroom were placed behind the sink, making them difficult for children to see and follow.
