110-300-0145(1) - A large, thick wood panel was observed near the fence, within the climbing equipment's fall zone. 110-300-0145(8) - The porch/berrier in front of the exit gate around the brick had gaps between the wooden railings wider than 5 inches; and a gap bigger than 10+ inch between the bottom wood section and the brick itself. 110-300-0145(11) - The self-latching mechanism on the gate was observed half-latches.
