2a) Toy bins that were accessible to 1 year old in care had small toys that fit in Safety 1st choking tube and posed choking hazard. 3c) Toilet handle was cracked 4f) Deck stairs that lead to outdoor play area were gated off from infants and toddlers but the bottom of stairs were not gated off.
