Observation

Licensor observed a long cord from the electric chair to be on the floor in licensed child care area accessible to children in care. Licensor observed a broken slide in outdoor space, that provider stated is being used by the children in her care. Licensor observed water temperature to be at 137 degrees Fahrenheit. Licensor observed outlet in bathroom near sink to be GFCI, but not tamper-resistant.
