Observation

110-300-0165(2)(c) preschool #1 classroom- there was a cord in the art area able to form a loop, a cord by the window hanged onto the curtain that was able to form a loop and in the far left corner a white cord able to form a loop; all accessible to the children. 110-300-0165(2)(d) Toddler #1 classroom- diapers inside plastic bag under sink accessible to children. Preschool #1 classroom- near cubby there was a plastic bag on the floor with sleeping matt inside. The restroom inside the classroom there was a plastic bag under the sink accessible. Toddler #2 classroom- the restroom in this classroom there was a plastic bag with diapers inside. 110-300-0165(3)(g)- the outdoor area there was two red bikes missing handle grips. 110-300-0165(5)(b) the outlets in the two restrooms near the sink are only tamper resistant- need to be tamper-resistant and GFCI. 110-300-0165(3)(f) Preschool #1 classroom- shelf on the right side end of the wall could easily be tipped over. Preschool #2 classroom- curtain hanger was not latched on to the wall- right side was hanging off.
