0165(2)(d) plastic bags accessible to children on the path to playground 0165(3)(c) wood cutter bench with a loose cord and pieces of wood were stored on the path to playground. 0165(3)(f) Unused chairs and playground equipment/materials were stored in the children's play space posing hazard if falling on children. 0165(3)(g) Some items on the playground equipment were duplicates and were in poor condition which should be removed from the playground
