110-300-0165(2)(c) Licensor observed cords that are capable of forming a loop around a child’s neck. 110-300-0165(2)(d) Licensor observed plastic bags that are accessible to children that pose a suffocation hazard. 110-300-0165(3)(a) Licensor observed potential cut, abrasion or puncture hazards in the school-age classroom and emergency exit hallway. 110-300-0165(3)(f) Licensor observed objects that pose a risk of falling or tipping. Shelves that are not secured in classrooms. 110-300-0165(3)(g) Licensor observed equipment in poor condition that is not repaired. Items that have loose parts, rusty parts, flaking paint or other dangers. 110-300-0165(4)(c) Licensor observed lighting in the toddler classroom not to have a shatter-resistant cover. 110-300-0165(4)(e) Licensor observed water temperature to be above 120 degrees Fahrenheit. Measurements taken with a DCYF provided thermometer.
