Observation

Sensory Table in the yard had standing water on the lid and water that had gotten into the sensory table and appeared to have been in there for awhile withe leaves and other debris in it. Cots were in the yard and were turned over with legs in the air and were a hazard. The water hose reel cart was turned upside down in the yard and the hose was not securely on the cart. Children's play items slide and play house in yard was not spaced 6 feet apart. Basketball hoop was not secure. Slide play equipment was observed to be on grass and artificial grass.
