(2)(b) Window blinds in the Spruce and Evergreen rooms had cords which could form loops. (2)(c) Electrical cords in the Spruce and Evergreen rooms were accessible. (2)(d) Plastic bags were accessible in the Spruce room in a cabinet that did not have a child proofing device on it. Plastic gloves were accessible in the bathroom. (3)(e) Green cover to water box where a hose was located on playground was not secure and posed a tripping hazard. -Balance beam located between a tree and the building posed a tripping hazard as children could fall into either surface and be injured. (3)(f) Shelves in Spruce, Evergreen and dramatic play rooms posed tipping hazards. Ladded in the Spruce room posed a tipping hazard (4)(c) Fluoresent lights in Spruce, Evergreen and dramatic play rooms to be checked to see if they are shatter resistant. (5)(a) Electrical outlets were not tamper resistant type. (5)(b) Electrical outlets within six feet of water were not both tamper resistant and GFCI.
