0106(3) - Staff member employed longer than 3 months has not completed CCB 30 hours. 0106(8) - Staff member has expired safe sleep training. 0106(10) - Unable to verify completion of BBP of some staff in Merit and/or on-site. 0106(11) - Unable to verify completion of CPR/FA of some staff in Merit and/or on-site. 0106(12) - Unablet to verify completion of FH cards in Merit and/or on-site. *Admin was given an additional 24 hours to see if they were able to gather documents prior to citation.
