0345(3) Based on staff interviews and video footage, it was confirmed that a child was able to leave the classroom and then the building unnoticed. 0345(5)(c)(vii) During recheck visit on 4/16/24, it was observed that teachers on the preschool playground did not have sight supervision of all children due to where the teachers had positioned themselves.
