0165(2)(b) There was windows blinds in the living room, room 1&2, and kitchen with string cords. 0165(3)(a) There was a door on the outdoor storage with splintered edges and mold that needs to be repaired 0165(3)(c) There was a toy outside with the plastic broken and cracked. There were some tires and trash bags and other debris in the outside area that need to be removed. 0165(3)(f) There were some bookshelves not anchored during the monitor visit. 0165(5)(a) The outlets in the living room and kitchen were not tamper-resistant. 0165(5)(a)(b) The outlets in the restroom and kitchen sink wall were not tamper-resistant & ground fault circuit interrupter (GFCI).
