Observation

Licensor observed some lose cables around wall areas. Some shelves were observed to be unstable and a risk of falling. Provider's water temperature was observed to be at 122 degrees Fahrenheit. Provider did not have a GFCI or tamper-resistant outlet. Provider had a cord plugged in that was not in good working condition.
