2c) Licensor observed two cords (a/c cord and light device cord) in preschool room that were accessible to children and capable of forming a loop 3f) Block shelf in toddler room was not secured to wall and capable of tipping 3g) Wall behind toddler bathroom had hole in wall that had chipping paint that was accessible to children 5a) Two outlets in preschool room were not tamper resistant and accessible to children
