2a) Provider had outlet covers in TR outlets in playspace that were capable of posing a choking hazard 2c) Infant swing had cord that was capable of forming a loop 2d) Plastic bags were found in lower bin in open diaper changing station in play area. 4e) Water temperature reached 125.6 degrees 5b) Outlet next to bathroom sink was GFCI but not appear to be tamper resistant.
