110-300-0165(2)(c) Licensee had cords accessible in the breakroom, and an extension cord by the primary prep exit door. 110-300-0165(2)(d) Licensee had plastic bags accessible in the primary one bathroom, and in the primary prep room, but all plastic bags were removed and made inaccessible on this site visit. 110-300-0165(3)(a) Licensee has a broken wood fencing in the outdoor play area with splintering edges, and one set of sharp yard shears were laying in the outdoor play area, the shears were removed during this site visit. 110-300-0165(3) (f) Licensee has two blue shelves in the entrance area, a large white shelves by the door, a set of brown cubbies, by the second exit, in the primary one classroom, in the hallway outside the commons area there were three brown shelves, and one large brown paper holder shelve that need to be anchored due to a tipping hazard. There was a plant that posed a tipping hazard risk in the primary prep 2 room, but it was removed from the window on this site visit and placed on the floor. 110-300-0165((5)(a) Licensee is missing having a tamper resistant outlet in the primary prep room by the exit door.
