0170(2)Provider states she has not filled out a fire inspction form since previous licensing visit which was longer than 12 months ago. 0170(3)(i) Provider does have a fire extinguisher, but states it was purchased 18 months ago and has not been serviced since. Fire extinguisher is kept in the window sill of the laundy room and not mounted to wall.
