- provider did not have a back-up battery for her phone in case of an emergency - the front door was locked and it was one of the emergency exits for the main floor - gates were locked at the top of the stairs - making it a two step process to go through and in the way of one exit - the window on the bathroom level of the home is over 44 inches from the interior floor and there was nothing to stand on
