0165(2)(c) Center did not have cords, phone cords, and computer mouse cords secured leaving cords accessible to children. 0165(2)(d) Center had plastic bags accessible to children in the bathroom on the side of the trash can. Center had plastic bags accessible to children in a closed cabinet on the left side of the sink area in room #2.
