On 5/13/2024, An epi pen was observed with the child's name taped to the only epi pen located in a locked black medical bag. No prescription box or prescription label with the child's first and last name name was observed with the medication. No way to identify who the medication was prescribed to without the prescription box with the prescription label. No prescription information with the date the medication was filled or the name of the prescribing health professional observed. No prescription box and prescription information with the dosage amount or length of time to give the medication observed. No written instructions for administration and storage of the medication observed with the medication.
