Observation

A heavy metal chain-link chain, was observed looped around a metal fence and locked with a paddle key lock. The metal heavy gaged chain observed forming a loop that could get around a Childs neck posing a safety hazard to children. Cracked slide and swing structure was observed in the outdoor area and removed by provider during the visit.
