Observation

(2)(a)(iii) Provider Higdon did not make a self-report of any kind to Children's Intake or to the Licensor when a child was injured during a struggle with the Provider to physcially move them to a time out chair on March 24, 2023. This incident was revealed by a later CPS Intake report, again not self-made by Provider Higdon, in May 2023. (3)(b) Provider Higdon also did not make or send a written incident report of the injury within 24 hours after the after the struggle on March 24, 2023.
