The provider removed all plastic during the monitor visit. 0165(2)(c) There were some phone cords on the kitchen counter during the monitor visit. The provider fixed all cords during the monitor visit. 0165(3)(c) There was a car toy missing steering wheel and a rocking horse missing handles during monitor visit.
