110-300-0170(3)(b) - Provider had shelf placed right infront of the fireplace, provider will move to another wall to ensure there is nothing blocking fireplace within 3 feet of it. 110-300-0170(3)(f) - Provider did not have fireplace/chimeny use form filled out. Copy sent and will wait for complete form to be sent back. 110-300-0170(3)(i) - Provider did not have copy of reciept attached to fire extinguisher, or servicing tag from Fire marshal.
