0210(1)(2) During the inspection, Licensor observed 4 childrens records with no immunizations on file. 0210(3) During the inspection, Licensor observed the 4 children s records without immunizations to not have a signed and dated statement detailing when the childs immunizations would be brought up to date and stating they understand their child will be excluded from care if the immunizations are not completed within the 30 days of the specified due date.
