Observation

0215 (3)(a)(ii) Provider had several containers of medicine in a white cabinet just to the right of the fridge, that was not labeled with child’s first and last name and did not have medical consent forms. One bottle had the first name of a child but not the last name and did not have the medical authorization form. 0215 (3)(c) Medication was stored in a cabinet with food.
