Observation

Licensor observed window blinds in kitchen. Kitchen is not licensed, but is used for food preparation. Provider stated that she has a gate, but during the time of visit, gate was not used and kitchen was still accessible to children. Licensor observed child enter the kitchen one time during visit. Licensor observed a tablet with two cables on top of it in sleeping / nap area. The cables were at child's level and accessible to them. Licensor observed 2 large, white cubbies with toys that were observed unstable with licensor lightly moved them with her hand. Licensor observed 2 mats in kitchen / dining area that were observed to have rips.
