2)b) There are window blinds in the front window that are accessible to children. They do not have a pull cord, but they do have inner cords that can be pulled out to form a loop, which has been determined to pose a hazard to children. 3)c) There was a broken riding toy in the toy box in the outdoor play area. One of the handlebars had broken off. 5)a) The electrical outlet cover on the outlet in the outdoor play area was not secured, and the cover could be opened by a child. 5)f) There is an extension cord being used to plug in the television in the playroom because the television cord is not long enough to reach the outlet. The extension cord is secured to the wall and is being used on an ongoing basis, not for a brief, temporary purpose.
