Observation

Provider Nashali Rivera had outlet covers that posed a choke hazard. Provider Nashali Rivera did not have stickers on sliding glass door at child's eye level to prevent walking into glass. Provider Nashali Rivera's water temperature in the restroom handwashing sink was hotter than 120 degrees. Provider Nashali Rivera had several outlets in licensed space that were not Tamper Resistant.
