During health and safety recheck, Licensor reviewed MERIT for staff member with initials (AG-- listed as Lead Teacher in MERIT). Staff member education has not been submitted for review at the time of the visit. (1)(b) During health and safety recheck, Licensor was not able to review staff file for the Provider. Provider's staff file must include: First and Last name, Date of Birth, Job title, First day of Employment, and trainings (2)(a)(b)(c)(d)(e) During health and safety recheck, Licensor was not able to review staff file for the Provider.
