2) The program continues to exceed capacity in licensed spaces, ratio and group size. 3) The staff struggle with meeting supervision requirements. Incidents continue to occurr without staff seeing, due to exceeding ratios and group sizes. There are too many children for staff to supervise at one time. 5b) the doors in the immediate area cannot be heard to open. Children were seen to leave spaces unnoticed.
