Observation

A child has a list of food allergies but did not have paperwork for all the allergies. There was only a Individual Health Care Plan for Peanuts. The Individual Plan was incomplete and lacked the signature of the doctor on the plan for the Epipen. The licensor also did not observe a medication authorization form for the Epipen. It was not clear if all the allergies were as severe as Peanuts but there is a list of food allergies. Form also mentions the use of vaseline with Epipen but it is unclear what the vaseline is used for and there was none present. Child is taking a daily dosage of Budesonide however the plan does not have doctor signatures on the medication forms and there was none present for the licensor to read the prescriptive information. Staff was unclear also what the medication given was for. Need to ensure that Epipen is labeled with child's first and last name along with any other medications that are brought in for this child or any other children.
