During onsite inspection, requested to review the onsite monthly fire evacuation drill record and Site Coordinator FB was unable to produce records. (4)(a) During onsite inspection, requested to review the onsite once every three calendar months whether an Earthquake, Shelter in Place, or Lockdown drill was completed, and Site Coordinator FB was unable to produce records.
