0291 (1)(d) Licensor observed infant room did not have sufficient lighting in the room to observe infants breathing or skin color while sleeping. 0291 (1)(h) Licensor observed infant sleep equipment was placed under a floating shelf that stored blankets that may pose a suffocation hazard when blankets tilt or fall on the sleeping infant.
