0165(2)(c) There was a cord loose on the kitchen counter. Provider removed the cord during the monitor visit. 0165(2)(d) There was some plastic bag on top of the cubbies in the living room, in the kitchen on bottom closed cabinets, and in the restroom accessible to children. The provider removed some of the plastics during the monitor visit. 0165(3)(a) There was a knife in a closed top cabinet access during the monitor visit. The provider moved the knife into a locked cabinet during the monitor visit. 0165(3)(f) There were some bookshelves not anchored during the monitor visit.
