0215 (3)(a)(i) Prescription medication on the counter and in the refrigerator for provider's granddaughter was not accompanied with medical authorization form. 0215 (3)(b) The provider does not have medication log for the medication she is administering to her grandchild. 0215(3)(c) There were several packages of medication on the dining table in the kitchen that belonged to the provider's granddaughter that were accessible to the children.
