0165 (2)(d) Licensor observed a box of diaper trash bags stored under the diaper changing table was accessible to children. 0165 (2)(f) Licensor observed that the sliding glass door lacked stickers to prevent children from walking into or through the glass. 0165 (3)(a) Licensor observed that knives were stored in the upper cabinet, which was accessible to school-age children. This presents a potential safety hazard, as children could be seriously injured if the knives were made accessible. 0165 (3)(e) Licensor observed an unrolled garden hose, which poses a tripping hazard during outdoor play. 0165 (3)(f) Licensor observed that the television monitor was mounted to the wall but not secured properly, allowing the arm to extend outward, which posed a risk of falling or tipping over. 0165 (5)(b) Licensor observed the outlet near the bathroom sink was not equipped with a GFCI outlet.
