Observation

During onsite inspection, Licensor observed an infant place an object in their mouth that posed a risk of choking (object measured less than one and third inches and was measured using a Safety 1 choking instrument), and six other objects werest observed on the floor near the infant that measure less than one and third inches near the infant. (2)(a) During onsite inspection, Licensor measured a 6-inch gap in the pressure gate that is installed on the front deck stairway. (4)(f)
